Chadham By The Sea requires that every guest staying in our building complete this form for our records (even if you already completed forms for your landlord or rental agent). This form is also your application for gate access to enter the parking lot and pool gate access to use the pool and pool deck. Please obtain a parking pass from your landlord or rental manager. Failure to display a valid parking permit while at Chadham by Sea may result in your vehicle being towed. If we don’t receive a Registration Form properly ﬁlled out by 12:00 noon on the last business day before your check in, you will not receive a gate code to give you access to the property nor access to the pool and pool deck.

RV's, trailers (except a boat trailer separately approved) are not allowed on the premises. There is limited boat parking, and you must request permission to bring a boat to Chadham By The Sea. If you have any questions please call your landlord or rental manager. Begin displaying your parking passes immediately while at Chadham by the Sea. If you have a state issued Handicap Permit, please display BOTH your Chadham by the Sea parking pass and Handicap Permit.

IMPORTANT: If anyone in your party needs "rescue assistance" due to a inability to descend the stairs in the event of a ﬁre, please indicate here and we will add their name and unit number to the list of people for the ﬁre department to assist. This information is kept conﬁdential. Local fire regulations restrict the use of elevators in a ﬁre emergency.

I hereby acknowledge that I have received a copy of the Chadham by the Sea Rules and Regulations (and if the rental agent or landlord is completing this for the renter, that the person completing this form will see that the renter receives this information) and that I will obey all of the rules and regulations of the condominium. I will take full responsibility for the actions of the remainder of those in my party as well as any guests I invite into Chadham by rhe Sea. I understand that I will be held liable for damages, disturbances, or any other improper action that occurs as a result of my occupancy of these premises. I will inform all occupants and guests that they are required to reveal their name and unit number to any Chadham by the Sea staff member or security ofﬁcer when asked, and failure to do so may result in eviction or arrest for trespass.
